The Private Healthcare Information Network (PHIN)

The Private Healthcare Information Network (PHIN) is the independent, government-mandated source of information about private healthcare, working to empower patients to make better-informed choices of care provider. This ensures that all patients considering private healthcare will have access to trustworthy, comprehensive information on both quality and price to help them make their decisions. In line with this legislation all patients will receive a pre-consultation letter which will provide information regarding my charges etc.

In accordance with recent legislation all patients will be sent information to help patients make informed treatment choices.

A letter will be sent out.

This letter sets out some important information that I am required by law to provide to you. This is for your information only and is not a bill. As this includes information about my charges if you do not have private medical insurance but someone else will be paying your bill, you may wish to pass a copy of this letter to them. Please note that even if someone else is paying your bill or you have private medical insurance, you are responsible for paying any charges which they do not pay.

Consultation

My fee for an initial consultation will not exceed £252 and my fee for a follow up consultation will not exceed £252. These estimates are correct as at the date of this letter. Should you cancel your appointment within less that 72 hours notice, a cancellation fee of £50 may be charged. This also applies to failure to attend without notification.

Following your consultation you may need certain tests (such as blood tests or imaging, for example an X-ray, MRI or CT scan) to help me diagnose your condition. If the test is undertaken by the hospital or clinic, the fees for those tests will be determined by the clinic or hospital and charged to you or your private medical insurer separately by them.

Private Medical Insurance

If you have private medical insurance, please contact your insurer before your consultation to check the terms of your policy, particularly the level and type of outpatient cover you have, including any reimbursement limits on individual consultation fee. Please note any excesses on your policy will be for your own account and will be payable to me on instruction from your insurance company advising you and me of this excess.

I am recognised by all major insurance companies.

Uninsured Patients

If you do not have private medical insurance, payment will be requested on the day of your consultation with me and an invoice/receipt will be sent to you by post.

Information for Insured Patients

Mr Ishaque is registered and recognised by all of the major insurance companies.

Patients who have Private Medical Insurance will need to contact their private health provider prior to their appointment and check the terms of their policy. This will ensure that they are covered for their initial consultation, scans, x-rays and any other treatment that may be required. The insurance company will provide patients with a Pre Authorisation Number or Claim Number, which will need to be provided for administration and invoicing purposes and will allow us to deal with the insurance company directly for the payment of invoices.

If there is an excess on the patient’s policy, the excess will be for the patient’s own account. Payments will need to be made to Mr Ishaque by cheque, credit or debit card in a timely manner.

Insured patients will require a referral letter from their GP, or another Healthcare Professional, this is stipulated by Private Medical Insurance Companies.

Information for Uninsured Patients

Mr Ishaque is happy to see uninsured patients in his private clinic. However it is important that uninsured patients appreciate that imaging (MRI and plain radiographs/or x-rays) may be required to achieve an initial diagnosis.  Consultation charges are available on request. Subsequent charges for imaging and if appropriate, surgical procedures may also be requested. 

A GP (or other Healthcare Professional) referral letter is requested; this will provide a medical history and ensure that the GP is kept informed.

Uninsured patients will be invoiced following their consultation and payment may be made by cash, cheque, credit or debit card.

BMI The Priory Hospital are able to offer package prices for surgical procedures should these be required.

Payments

Terms and Conditions

All patients seen by Mr Ishaque are required to provide valid credit/debit card details prior to being seen.

The Clinic works with BMI and Wolverhampton Nuffield Hospital but is a separate entity. The card information you register with us will not be shared with anyone else, including the Hospital. Your details will be securely stored and destroyed following completion of your treatment and full settlement of your account.

No payment will be taken from your card without your prior permission 

Uninsured patients

If you are a self-funding patient, payment will be debited from your card following your consultations and an invoice and receipt of payment will be sent to you in the post. If you would prefer to settle your consultation charge on the day by cash or cheque directly to Mr Ishaque, please contact Linda Hamilton on 0121 446 1674 to confirm this method of payment.

Professional Memberships

Mr Ishaque is highly experienced and widely recognised as a leading Consultant Spinal Surgeon. He is one of the few surgeons to have been awarded both the British Orthopaedic Association's Robert Jones Gold Medal and a Hunterian Professorship from The Royal College of Surgeons of England. He is one of the youngest surgeons to have achieved this, having been awarded both honours, before the age of 40.
